Dr. Ileana Ponce-Gonzalez is going to interview 1) Patrick Suther, the CarePort Program Manager, 2) Daniel Hannawalt-Morales, the Community Care Hub Director; and 3) JP Anderson, Chief Executive Officer of CHOICE, (Community Health Organization Improving Care and Equity) is a nonprofit organization dedicated to promoting health equity, and improving healthcare access, outcomes, and systems of care. We collaborate with local healthcare providers, community partners, and Tribal health leaders to fund and support regional health improvement projects.
